For EMPLOYER Clients:

If you are sending in an employee for injury treatment or pre-employment testing services, download the necessary TREATMENT AUTHORIZATION AND PATIENT FORMS.

CCS is a free service available to all our U.S. HealthWorks clients. You’ll be given 24/7 on the web access to your injured's case status and history as well as reports that can be customized for your company such as summarized case statistics and more.
