CONTEST RULES and REGULATIONS:

1. No purchase necessary to enter the U.S. HealthWorks Twitter contest. The contest will end on Feb. 8, 2010. The opportunity to enter the contest may be affected by an entrant’s ability to access the Internet at any particular time.
2. The U.S. HealthWorks contest is open to anyone 18 years or older who are legally allowed to participate in such a contest as allowed by their local laws.
3. Entrants must become a follower of @USHealthWorks on Twitter, ReTweet the offer at least once AND post a comment on the blog as to why their office deserves to be vaccinated from H1N1. U.S. HealthWorks assumes no liability for the inability to participate in this contest due to any telecommunications malfunctions or inaccessibility to @USHealthWorks on Twitter, the ability to ReTweet or to post a blog for any reason whether or not due to the fault or other failure of U.S. HealthWorks or any third party. The entry judged by U.S. HealthWorks in its sole discretion to be the best will win the inoculation visit for their business.
4. U.S. HealthWorks reserves the right to cancel or modify the contest if fraud, voting irregularities, manipulation of results or other inappropriate conduct, destroys the integrity of the game as determined by U.S. HealthWorks in its sole discretion. All comments become the property of U.S. HealthWorks and may be used for promotional purposes.
5. Employees and families of U.S. HealthWorks and its affiliates and subsidiaries are not eligible to participate in this contest. Participation in the contest is void where prohibited by law. All Federal, state and local laws and regulations apply.
6. The Entrant who wins will be informed at the close of the contest. U.S. HealthWorks assumes no responsibility or liability for any damages, losses, or injury resulting from the acceptance of the H1N1 vaccination. Further, appropriate medical authorization forms must be completed in full at the clinic by each employee of entrant who chooses to be inoculated prior to any H1N1 vaccination in accordance with all medical protocols now existing or that may exist at the time of receipt of such vaccination.
7. The Winners place of business where H1N1 vaccinations will be administered, must be within 20 miles of one of our U.S. HealthWorks Medical centers where H1N1 vaccine is available.
8. Acceptance of the prize offered constitutes permission to use the winner’s company name and/or likeness for purposes of advertising and trade, worldwide in perpetuity, without future compensation, unless prohibited by law. Entering the contest is considered a confirmation of eligibility on behalf of the entrant in accord with these rules and any pertaining federal, state and local laws and regulations.
